A birth registration form is used to register a live birth born for women who are members of the
DSS. Live birth is defined as a pregnancy outcome that showed signs of life, such as movement or
crying, after being delivered. Irrespective of how long they lived after birth, all children who were born
alive should be recorded using this form. Note that there might be a tendency for some women to
report a pregnancy outcome whose child died immediately after birth as a stillbirth; this should be a
live birth. You should also be aware of some cultural practices that may affect a response to this
question. For instance, in some cultures, a child is not considered as a “human being” until after
certain rituals are conducted. Appropriate questions needed to link the birth with its mother should
be included. Because the new born is going to be considered as DSS member, applicable
questions from base line census questionnaire should be asked.
